Enrollment Associate - Patient Enrollment Team- Full Time.
Position Overview: As a key member of our team, you will be the first point of contact for individuals interested in our functional medicine services. This role is built for someone who thrives on hitting targets and managing a pipeline with precision — you'll own the full lead-to-enrollment process, tracking every inquiry from first contact through active patient status. You'll educate potential patients on how our programs can help them recover from chronic illness, achieve their goals in our wellness program, or join our primary care program, all while working toward clear enrollment and conversion goals alongside a passionate, supportive team.
What You'll Do:
- Own Your Pipeline: Manage incoming inquiries (phone, email, online forms, word-of-mouth) in our CRM, keeping every lead accurately logged, categorized, and moving through a defined follow-up cadence.
- Initial Contact: Reach out to potential patients promptly, assess their needs, and provide clear, structured education about how our programs can support their health goals.
- Program Enrollment: Match potential patients to the right program, clearly explain benefits and costs, and guide them through enrollment — tracking conversion from inquiry to enrolled patient.
- Hit Enrollment Targets: Work toward monthly enrollment goals, using CRM data to identify where leads are stalling and adjust your follow-up approach accordingly.
- Coordinate Care: Once enrolled, handle the administrative details precisely — scheduling initial appointments, collecting required forms, and confirming every step is complete before handoff.
- Collaboration: Work closely with clinical staff to ensure a seamless, well-documented transition into ongoing care.
- Hybrid Work: Enjoy a hybrid work schedule, with a combination of remote and in-office days, along with meetings with the Enrollment Team and other SFM team members, monthly or quarterly in-person meetings with the whole staff.
What We're Looking For:
- Goal-Driven, Detail-Oriented Track Record: Previous sales or customer service experience with a demonstrated history of hitting targets — you track your own numbers, notice trends before they're pointed out to you, and don't let a lead go cold.
- Passion for Wellness: A genuine desire to help others heal and improve their lives through functional medicine.
- Systems Thinking: Comfortable managing a high volume of leads across multiple stages at once without letting details slip — you build your own follow-up systems rather than waiting to be told what's next.
- Communication Skills: Strong verbal and written communication, with the ability to explain complex concepts clearly and persuasively.
- Tech-Savvy: Comfortable using CRM software (experience with Active Campaign is a plus), email, Microsoft, and other online communication tools.
